It's Kihara-san's previous lover, the one who looks like Shoukichi. I suppose when he died, Kihara-san then met Shoukichi and started obsessing over him

It's actually not. Fuku made Shoukichi think that Kihara-san was more messed up than he actually was. Kihara-san wasn't that bad tbh (except for that part in the rain, that was scary o.o)
