I disagree with the opinions saying satsuki is transphobic but I do understand where they're coming from to say that.
But to me, since he avoided having a relationship with almost everyone in his life he didn't understand that what he felt had nothing to do with whether he saw him as a girl or a guy because I think falling in love doesn't need a gender to justify being attracted to someone, he felt bad for it cause he thought that being attracted to him he must think that he's a girl but it's really just love doing it's thing.
