Idt he can read minds all the time. There's plenty Adrian thinks that he's not clued into. Maybe it's a hyper-empathy thing with the condition of recalling something very painful/sad. It stands to reason that Harper can only respond with actions/words regarding extremely strong memories eg. The melancholia+love+guilt of loss with patroculus and the ocean, him knowing the woman next to the cafe was going to be evicted, the memory of patroculus burning.

In any case this is very cute and funny.
