I’m not sure if this is accurate since this is my own interpretation- but I think the Uke was like: it hurt to fill in the tattoo. Every time I thought of you, it felt like I was getting a tattoo = it hurts. He settled for only putting numbers on the tattoo every year, a number for every year he thought of him. So during the 7 years they were apart, there wasn’t a single year he didn’t think of him, he was always on his mind and heart.
