In the novel it is explained that there are two main reasons why he doesn't care much for his health.

First, his life has been awful. The love of his life and his parents treat him like trash, no matter what he does and how well he treats them (his lover because of that stu*** misunderstanding and because he was full of hate, mostly for the death of his parents; and his parents (the ones of the white hair one) just because they are trash, really, just thinking about money). So, he literally says: "It is not bad if I die. I'm so tired, this life is painful"

Second, when he was a teenager, he had to take care of the family business because his father led them to bankruptcy. Since he is 18 years old he has been the support of his family(parents and two little siblings) and his lover(because he loves him so much and wants him to be successful, even if he doesn't appreciate it. Plus, he feels kinda guilty for the death of his lover's parents; it wasn't his fault, but he witnessed, and thinks he could have done something (he couldn't have done anything, but he is so gentle and caring that he still thinks he could)).

Anyway, he has been providing and helping them for so long, that he would feel bad if he stops doing it. Even if they treat him like trash for no reason or for misunderstandings, in his heart they are still his family and the man he loves. So, he is scared of taking a full exam that reveals he cannot continue being the man who is there for them and who provide for them(in a direct or indirect way). For him that would be the same of becoming a useless person, and he hates that idea (plus, it makes him feel even more depressed), so he prefers to continue his life as it is until he just dies.

But, don't worry, the brown hair dude is gonna help him overcome his depression and fears, and to take care of him. So, little by little, he is gonna become interested in taking care of his health. Although, it will be a tough process.

Sorry for the huge spoiler, but I'm kinda angry because the novel, in my opinion, is beautiful, actually is my favorite novel; but, for some reason, the author of the comic is leaving out a lot of the story, like she is just running to finish it quickly. That's why the story feels full of gaps. ヽ(`Д´)ﾉ

I totally understand you! Actually, I was also so frustrated because he is my favorite character and I didn't wanna see him suffer. Alsoo, he is sooooooo stubborn, always wanted to appear strong (even when he is so sick ┗( T﹏T )┛). But don't worry! His boy (the brown hair one) is soooo dedicated to him after he reincarnated and will make him change his mindset.... but LITTLE BY LITTLE, he (the white hair one) is not an easy one (￣∇￣").

However, let me tell you so you can be prepared when you decide to continue the novel... although it has a happy ending, there will be WAY MORE SUFFERING, BUT, they are gonna be there for each other through all the hardships, that is a good thing, jejeje

In the novel, the lover has a character development, definitely. However, taking into account all the parts of the story the author of the comic is leaving out, I don't think we will see all the process of development in him, or even the white hair one, just the final result, which I hate (╯°Д °)╯╧╧.

They STAY TOGETHER! And, although there are not more leads properly, the white hair one is so interesting, successful and handsome that it will attract a lot of people, for envy, jealousy.... well, he is notorious so there will be problems because of that (￣∇￣")
