It's been a year since 141108 was published, but there are quite a few people who are still confused about the story so here's an official explanation by the author (though writing it and having to explain it again probably means I've epically failed already ;;) .

The characters in the story, Lam (dark hair colour) and Blanche (light hair colour) are lovers. They were both severly injured in a car accident, since both were in critical conditions, doctors decided to transplant Blanche's head onto Lam's body.
Therefore:
The one to control the body is Blanche
And Lam, theoretically, she's dead.
BECAUSE THEY WERE IN A RELATIONSHIP, naturally, Blanche was devastated. The new alter personality that has formed within her, because of the traumatic experiences, very closely resembles Lam (one of the similarities was that she also loved Blanche the way Lam did).
The detail about the pill, that's a sleeping pill. Hypothetically, the alter personality wakes up at a certain time of day, then after sleeping, Blanche returns to normal (I made up this detail because obviously alter personalities of dissociative identity disorder aren't controlled that easily ;___; )
Blanche becomes depressed and unstable, this explains the series of attempts to commit suicide.

About Blanche slashing her face, try putting yourself in her position, everyday you stare at your own reflection in the mirror, your face on the body of the one you love_dead, you will understand why she would hate it, wanting to destroy it. (I purposely drew lots of broken mirrors in their house in the story, Blanche smashed them because SHE HATES TO SEE HER OWN FACE IN THE MIRROR).

So what role does the doctor play? He's a side character, to an extent that I didn't even bother to draw his face. His role is for the alter personality to recognise the strange details and progress the psychological development of the character to climax. You can tell that he doesn't like Blanche's alter personality very much, but speaks very endearingly to Blanche. This depends on your interpretation, i wrote it that way because I wanted to make the conflict of him loving Blanche but despising the alter personality, which messes up her life even more (it wasn't an important detail initially anyways).
Oh and also the role of giving her the sleeping pills.

Lam is dead.
But to Blanche, the other personality that lives within her is Lam,
TO BLANCHE ALONE, LAM IS STILL ALIVE.
I wanted to make that kind of twisted happy ending.

To be honest, I would agree that the narration is quite hard to understand and I'm exaggerating quite a bit, but to me, just reading and understanding it straight away makes the story boring. I hope you're not too frustrated with my weird logic.

Almost forgot, the name 141108 is the date (08/11/14) that the Sergio Canavero's book "Immortal: Why CONSCIOUSNESS is NOT in the BRAIN" was published, the founder of the investigation into human head transplant. THIS IS A REAL INVESTIGATIVE PROJECT, they're planning to carry it out in 2017.
