Lam is the name of Blanche's dead lover. The doctor dislikes the other personality (Lam) but likes Blanche. Some people say he loved Blanche, but I think he was just doing that because he doesn't consider the alter-personality a human. (that's up to you). He probably told Blanche what happened to her and that's why she started to self-harm but didn't tell the other personality since like I said before, he doesn't consider her a person.
