Sometimes authors put stuff like this in the last panels to shock the readers and to read the next chapter. It may not be incest, it could be looking after a child together or something else.
Second of all, it is not entirely weird for the time of the manga, nobles married their cousins and half siblings too so not entirely mind breaking as uke thinks.
So calm down, even if it is incest it has nothing to do with the MAIN couple and its just introduced as a theme. Regardless if you are uncomfortable you have every right not to read but please for the love of God some of us like to thirst here and discuss so panic under one single post.
