I've been betrayed too many times with stories suddenly introducing romance between step siblings and stuff like that and the title is a little sus...
Is it referring to her step sisters or does the ML become her step sibling as well?

    No it doesn’t, there are no step brothers in the story only step sisters so you don’t have to worry about any incest since it’s not yuri either
