The Taran family bloodline work like this, the male can't have a child if not with their half-sister or a specific female, so for example, the duke are a Taran and the duchess is also a Taran because she's the duke half-sister, now when they have sex and produce the heir (the child will be 100% boy) and for the taran bloodline to continue obviously they need to have a girl for the heir to have child with in the future so usually their family doctor already prepare a specific female for the duke to have a child with(the child will be 100% girl), now this female from young age already got 'purify' (consuming a mugwort herb) which will stop her menstruation and (I forgot if there a specific time or that they just need to eat it until they turn 18 or so) after that they will get the cure and ready to get pregnant by the duke and voila the half-sister for the heir is ready and the cycles continued (usually the brother and sister got separate too and then later got reunite again, sometimes they know that they're sibling, sometime they don't know until later)!!....
