The first part is when Tachibana died and the phantom manuscript is with Asakura and awaits his death. The last part is to show they really did grow old together.

I kinda feel like the second half is an alternate ending because it essentially uses the a lot of the same panels from the first part, but changes the dialogue :
The second part is alternate ending. There were 2 different epilogues in tankobon and magazine. This is what scanlator said.
