Successors in hospitals just like in other jobs don't have to be related, people just force their kids to work on things similar and follow in the family footsteps from a very young age so they have a higher chance of getting out into a successor category without backlash from other doctors saying "This candidate is better" which is why rat man and his son hate jihyun because his existence creates that backlash so easily. Nepotism at it's finest.
