Yes, a lot of yaoi is pure sexual or romantic fantasy, and not realy meant to be very realistic. Other kinds mix elements of realistic relationship issues with fantastic settings. And then there are those which try to be mostly realistic.
I try to appreciate and enjoy each manga on its own merits. There is pleasure to be had in all kinds. :)
