lol it was a holy bullet lodged in his heart and the reason why it "reacted" when Gyeong Joon was around was due to the fact that he was in love with him. Cain thought it was a spell of some sort but it was just him fallin' in love and his heart going "doki doki." The reason why it was sometimes described as bein' painful was probably due to the bullet.
