So it seems. The extra takes place 10 years after high school so we can conclude Midorima is back and that everything is fine between him and Takao. Plus, after Takao came with the revived puppy, Midorima talk about adopting it and search for a house.

and if you look closely Midorima and Takao wears both ring at their left hand (Hayama and Miyagi too ^^).

at the extra chapter, midorima's back right? and all is cool with him and takao again right? i like to think of it that way because that's the only way i can calm my heart after tearing up. i know the feeling of leaving home— may it be a place or a group of people. so it must've brought the both of them immense pain.
