They aren't related. The guy is the demon who put himself in the womb and was born along side the girl because the mother prayed to the demon that serves her side of the family, (I forget what her wish was) so yes, they shared a womb. By reading the comments, I'm somewhat reading that both brothers aren't actually related to the FL? And is the FL the only child connected to the family bloodline? Chile... I'm so confused. Also budget Levi is hella insecure smh get him some milk
