Omega goes into heat because he ran out of suppressants and his friend who is an Alpha mated with him because of pheromones and all that jazz. The Alpha confesses his feelings and the Omega accepts him, but it turns out the Alpha is the one who threw away the suppressants so that the Omega would go into heat and he can take advantage of him.
